Answer 'Yes' to Preliminary Question
3 of the online application if you wish to apply for a Diversity
Fellowship. The deadline for receipt of complete application materials,
if you are requesting this type of financial support (including Diversity Fellowships),
is January 15, unless an earlier deadline date has been set by the
department. The Eugene Cota-Robles Fellowship Program is a merit-based
diversity enhancement fellowship program that provides financial
support to assist students from diverse backgrounds to successfully
pursue and complete a graduate degree. This fellowship is awarded
to students who have overcome significant obstacles to achieve a
baccalaureate level degree, and whose economic, educational, or
social background contributes to the intellectual diversity among
the graduate student population.

Statement of
purpose
This should be a concise, well-written
essay about your background and your reasons for pursuing graduate
study in the field you have chosen. Selection committees place particular
importance on the statement of purpose. It exhibits your ability
to present ideas in clear, coherent language. Your statement of
purpose should indicate:
- How knowledgeable you are in the
desired field of study
- How your undergraduate studies
and other experiences (work, community involvement, and so forth)
serve as a foundation for graduate study
- How and why you intend to build
on this foundation of knowledge and apply your graduate training
to social or theoretical problems
Personal History Statement
The Personal History Statement is required of all applicants.This statement will be used in conjunction with your application for graduate admission and financial support. Please note that the Personal History Statement should not duplicate the Statement of Purpose.
UC Santa Cruz is interested in a diverse and inclusive graduate student population. In an essay, discuss how your personal background informs your decision to pursue a graduate degree. Please include any educational, familial, cultural, economic, or social experiences, challenges, or opportunities relevant to your academic journey; how you might contribute to social or cultural diversity within your chosen field; and/or how you might serve underrepresented segments of society with your degree.

Fee
Your completed application must be
accompanied by a $70 application fee. Electronic payment options are available through our online application system and, when necessary, paper checks or money orders are accepted.
Fee waivers for cases of hardship are available to US citizens and permanent residents only; international students are not eligible. Funds for waivers are limited, but if you feel you qualify for a waiver, complete the “Request for Graduate Application Fee Waiver” form and return it to the Graduate Admissions office for approval. Do not submit both a fee waiver request and an application fee payment. If your request for fee waiver is not approved, you will receive an email advising you and requesting the application fee.
Fee waiver request forms must be received by the Graduate Admissions Office a minimum of 10 days prior to the department deadline.

When listing educational institutions attended
on page one of the application form, please include every college-level
institution you have attended, regardless of length of time in attendance,
and whether academic course work was completed or not. One official
transcript from the Registrar of each institution listed (including
university correspondence or extension divisions), or a statement
of good standing, if work was not completed, is required. Each transcript should
be in an envelope sealed and signed by the Registrar.
If you attended UC Santa Cruz as an undergraduate, you must also request and pay to have an official transcript sent to Graduate Application Processing from the Registrar's Office; they will not be available to us otherwise.
Transcripts submitted to UCSC will not be returned to the applicant, nor will copies be forwarded to any other persons or agency.
To qualify for graduate study at UC Santa Cruz, you must present
official evidence that you have received a bachelor's degree from
an accredited institution of higher education. (However, verification
of UC Santa Cruz bachelor's degrees can usually be accomplished
electronically.)

LETTERS OF RECOMMENDATION |
Three letters of recommendation are required. Additional letters will be reviewed, but will not help or hurt your application.
Letters of recommendation must be submitted electronically. You must register your recommender(s) on Step 4 of the online application. An email will then be sent to your recommender(s) with document upload instructions. Be sure to have your recommender add our messages@notification.embark.com email address to their email address books so the recommender alert e-mail doesn't get marked as spam by their email provider.
Letters should be prepared by professors, professional contacts, or others who may best speak to your abilities and academic potential in your chosen field of study.
You have the option of submitting a Waiver of Access Form. Once your letters of recommendation have been submitted, you are not permitted to inspect them; however, if you are admitted and enroll, you will have access to your letters of recommendation unless you have previously waived that right.

GRE scores must be received by the Division of Graduate Studies prior
to the application deadline. Have the Educational Testing Service forward
your test scores electronically to the UC Santa Cruz Graduate Studies Office
(School Code 4860). It is not necessary to indicate a department
code since all scores are sent to the Graduate Studies Office. Hard copies of score reports will be discarded, only electronic scores will be accepted.
Computer-Based Testing: Scores reports
are mailed to you and up to 4 institutions, if you designate them on the
day of the test, approximately 10 to 15 days after your test date.
Paper-Based Testing: Scores reports are

Reactivating an old application
Applicants have the option of reactivating an application from last year. You will need to submit a new application and fee, but you may use any or all of your supporting materials such as test scores, transcripts and letters of recommendation. Please note, we keep application materials for only one year, so if you applied two or more years ago, you will need to submit all your supporting materials again.
If you created an online application last year, access your Embark account (formerly Princeton Review), and all of your 2008 application information will be there. Please review the entire application before submitting for Fall 2009, as some pieces are not brought over (i.e. Statement of Purpose essay, Writing Samples) and we have made changes to the residency questions. In addition, no letters of recommendation are brought forward from the previous year. Once you have submitted your 2009 application, contact gradadm@ucsc.edu and specify which supplemental materials (transcripts, letters of recommendation, etc.) you wish to apply to your 2009 application.

All official academic records must be issued in the original language and accompanied by English translations prepared by the issuing institution. If translations are not available from the institution itself, translations may be prepared by government or official translators. In order for translations to be acceptable, they must bear the stamp or seal of the issuing institution or government agency and the original signature of the translator. | Residency Information |
Residency Information for Tuition Purposes
Adult students (over age 18) may establish residence for tuition purposes if they are a U.S. citizen, a permanent resident, or other non-citizen who is not precluded from establishing a domicile in the United States; and they meet the criteria described in the links below. Students who do not qualify as California residents must pay a Nonresident Tuition Fee in addition to all other fees (see graduate Fees and Expenses). The residence determination date is the day instruction begins at the last of the University of California campuses to open for the quarter.
If you have not been living in California with intent to make it your permanent home for more than one year immediately before the residence determination date for each term in which you propose to attend the university, you must pay a Nonresident Tuition Fee in addition to all other fees (see pages 19 and 51).
